   On the date of the visit, there were three teachers supervising twelve toddlers in a single toddler classroom. It was reported that two toddler classrooms had been combined due to staff being on leave or vacation. The program must submit a plan of compliance to ensure staff-to-child ratios are consistently maintained. Corrective action: On this day, we had an unusual circumstance of one teacher away on vacation and suddenly two others had medical emergencies that caused them to need to leave work. An arrangement was made that still allowed for one staff for four children but did not meet other rules for class size. In the future if a similar situation occurs, we will close the class for the day and notify EEC. Now that our PreK class graduated, we have less children which will help this matter..    1- In one of the bathrooms, the border around the floor is broken and needs repair. 2- The floor in the cafeteria area shows significant wear and tear, appears dirty, and has multiple cracks, making it difficult to clean. Corrective action: The floor in the lunchroom is cleaned and sanitized every day. We are going to replace it as part of our renovations plan. It is a huge floor and has already been measured for the new floor and we have received an estimate. This will be combined with other renovation work that we are having done. The time of finishing the project is estimated.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children.    The building inspection certificate was expired at the time of the visit. Corrective action: We did lose track of our expiration date this year. It has now taken us a while to get an appointment due to [redacted] being so backed up because of all the construction going on in the city. We do now have an appointment for September 18 or sooner if there is an opening.. Regulation: Building Inspection.    1- The playground equipment structure has multiple pillars that are cracked or broken. 2- One area of the fence has an opening that poses a potential entrapment hazard. 3- The spring rocker has visible damage, including worn paint, chipped edges, peeling coating, and splintering or fraying of the underlying material. The red side panels have large areas where the paint and protective coating have worn away. Corrective action: The playground equipment has all been repaired or replaced. In the future it will be inspected daily, and repair arrangements will be made right away. And as always children are not allowed to use damaged equipment. This will all be reviewed in a staff meeting.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space.    7.07(06) — Physical Facility - Outdoors

   Peeling and chipping paint, as well as broken plaster, were observed in multiple areas both inside and outside the facility. This included: 1- Interior classroom walls- Cracks were observed in the archway of the preschool classroom 2-Exterior window sills and trim Corrective action: Preschool Classroom Archway and walls repaired. Also, glass trim on windows repaired. Cracks, broken plaster, chips etc. are cleaned up every day. And ongoing paint and plaster repair inside and outside are estimated to be finished by 10/18/2025. Regulation: Chipping and Peeling Paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.    The allergies and medications list did not include all enrolled children with documented allergies and/or prescribed medications. Corrective action: One child had a physical that stated that the child had an [redacted] allergy. The child's [redacted] insisted that [redacted] did not have an allergy which turned out to be correct We were waiting for verification from the doctor's office. Staff were notified of the possible allergy, but it was not written on the ALERT List yet. In the future a note will be put on the Alert List stating that the situation is "pending", as the licensor recommended..    7.11(05)(d)(04) — Health

   The program did not have the prescribed medications available for some children as required by their Individual Health Care Plans (IHCPs). Corrective action: The children that did not have their medications available at the school in both cases the children's parents had not yet received these new medications from their pharmacies. So, we could not have had the medication at our program yet. The licensor recommended that we attach a note saying "pending", to the IHCP until the medication is brought into our program. We have done this. And will in the future..
