The preschool classroom had 12 children in attendance, 10 children signed in. The attendance was kept outside the classroom in hallway and periodically checked for accuracy by teachers. Corrective action: To ensure accurate attendance records and compliance with EEC regulations, Gan Keshet Preschool will implement the following corrective actions: Family Reminder The Director will send a written reminder to all families emphasizing the importance of signing their child in upon arrival and signing their child out upon departure each day. Teacher Verification at Arrival Teachers will actively connect with families during drop-off to verify that children have been signed in on the parent attendance log. If a family forgets to sign their child in, teachers will remind them to do so immediately. Classroom Attendance Tracking Teachers will maintain a separate classroom attendance log to ensure an accurate accounting of all children present in the group/in the classroom. Teachers will reconcile the classroom attendance log with the parent attendance log and promptly update the parent attendance record if a sign-in was inadvertently missed. Attendance Records Accompanying the Group Attendance clipboards will continue to accompany each group throughout the day and will be carried by teachers whenever children travel to different locations, including outdoor classrooms, the forest classroom, the farm, and other program spaces. Teachers will reference attendance records during transitions and conduct regular headcounts to ensure all children are accounted for. Teacher Verification at Departure Teachers will ensure that families sign their children out before departing for the day. Teachers will also record departures on their classroom attendance log to maintain consistency between attendance records and ensure accurate documentation of children's whereabouts. Ongoing Monitoring The Director will review attendance procedures and records to ensure compliance with EEC regulations and to identify any areas requiring additional support or training. These measures will help ensure accurate attendance documentation, improve accountability, and support the safety and supervision of all children enrolled in the program.. The building and fire inspections are expired. Corrective action: Building and fire inspectors came to complete inspection on 7/11/25.. Regulation: Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). In the outdoor space, one picket is missing on the wood fence and it poses a head entrapment hazard and in another section of fence, a board is missing on top of the fence and there are three protruding screws. Corrective action: Fence picket was repaired/area patched to prevent head entrapment. Three protruding screws were removed by our head of maintenance.. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space.
