K.A.R. 28-4-115 Facility. (c) ( 5) is uncluttered, visibly clean, and free from any evidence of vermin infestation and any objects or materials that constitute a danger to children in care;

Cleanliness is not being maintained on the premises. There are multiple buckets of laundry throughout the facility. Floors are covered in food from previous meals such as tortilla chips, mac n cheese, and other unidentifiable food. Items such as toys, books, stuffed animals, craft supplies, etc. are in disarray on the floor in an unkempt manner. Cardboard, candy wrappers, and pieces of paper are laying around the floor of the facility. Kitchen counters are covered with items, limiting the available counter space for food preparation. Mounds of clutter pose tripping hazards in walkways of the facility. Dining table where children eat contain food remnants and other spots of grime. K.A.R. 28-4-115 Facility. (c) ( 8) has each electrical outlet covered or inaccessible to prevent easy access by a child when the outlet is not in use;

K.A.R. 28-4-115 Facility. (g) (1) All household supplies, cleaning supplies, dangerous chemicals, and all bodily care products containing alcohol or bearing warning labels to keep out of reach of children shall be in locked storage or stored out of reach of children under six years of age. Soap used for hand washing may be kept unlocked and placed on the back of the counter by a sink.

K.A.R. 28-4-116 Daily care of children. (d) (11) Tableware shall be washed, rinsed, and air-dried or placed in a dishwasher after each meal.

Lunch plates containing food were located on the dining room table. Other unrinsed dishes and bowls were located in the sink. Two high chairs have not been cleaned, as there is still food on the chair and tray. K.A.R. 28-4-117 Health care requirements for children under 16 years of age and recordkeeping. (c) Immunizations for each child, including each child of the provider under 16 years of age shall be current as medically appropriate and shall be maintained current for protection from the diseases specified in K.A.R. 28-1-20(d). A record of each child's immunizations shall be obtained not later than 60 calendar days after the child's initial enrollment in a child care facility and shall be maintained on the child's medical record.

K.A.R. 28-4-115 Facility. (c) (17) has interior and exterior surfaces of the facility that are free from peeling, chipping, cracking, scaling, and loose paint.

Interior rooms of the facility have multiple areas where paint is peeling and chipping on the baseboards and wall. Pieces of paint are scattered on the floor accessible to children. K.A.R. 28-4-115 Facility. (i) (2) Outdoor play equipment that is safely constructed and in good repair shall be available and placed in an area free of health, safety, and environmental hazards.

K.A.R. 28-4-124 Parental permission for children to go off-premises. (b) Written permission on a form supplied by the department shall be obtained from the parent or legal guardian of each child who will be transported or allowed to participate in any off-premises field trip.
