Soap used for hand washing may be kept unlocked and placed on the back of the counter by a bathroom or sink.

License capacity for the Mixed Age unit was exceeded when there were 11 children present in the unit. The unit is licensed for 9 children. K.A.R. 28-4-132 Child care practices. (a) Each applicant, each applicant with a temporary permit, and each licensee shall ensure that supervision is provided by a person 16 years of age or older who is responsible for the health, safety, and well-being of each child in care.

Appropriate supervision was not provided by staff in the Preschool Unit. Three children were observed opening and closing the front door to the facility and three children went into a bathroom together closing the door, the one staff present in the unit was sitting on the floor in the back corner of the unit, resulting in staff being unable to visually supervise or intervene. In the Preschool Unit, several children were observed climbing on top of tables, chairs, and shelving units throughout lunchtime. Staff were serving and cleaning up during this time and were present and not actively supervising or intervening. Appropriate supervision was not provided by staff in the Mixed Age Unit. Two children were observed climbing a gate/barrier, which separates play space from sleep space, and climbing into a playpen together while staff were present and not actively supervising or intervening. The activities, supplies, and equipment shall be designed to promote the following:

Planned learning experiences and daily routine are not followed leading to a lack of routine and a chaotic classroom environment that does not promote healthy growth and development. As evidenced by children climbing on top of tables, chairs, and shelving units, running throughout the units while screaming and crying, opening and closing front door, and climbing barriers into sleep space. During lunch in the Preschool Unit, staff permitted children to take blueberries directly with their hands from a shared bowl that was passed around. During lunch in the Preschool Unit, a child placed the serving tongs from the blueberry bowl into their mouth.
