The backyard outdoor play area contains numerous health, safety, and environmental hazards. A nonoperating riding lawnmower and a nonoperating car are located adjacent to the outdoor play structure. The car is missing a front tire/wheel, is sitting on a block, and the hood is ajar. An excessive amount of tree limbs and twigs are piled throughout the yard and on the back porch. A chainsaw, three hammers and a lengthy extension cord are within reach of children on the back porch. A child size plastic picnic table and infant activity seat are heavily soiled with dirt from being left in the elements. An infant seat and play mat located on the back porch are heavily soiled from being left in the elements. Multiple interior and exterior surfaces of the wooden outdoor child size playhouse have an excessive amount of curling, peeling and chipping paint. K.A.R. 28-4-116a Napping and sleeping. (c) (9) When not in use, cribs, cots, playpens, pads, and bedding shall be stored in a clean and sanitary manner.

(e) If an infant who has not been immunized against measles, mumps, rubella, and varicella because of the age of that child is enrolled and there are children in care who have not had measles, mumps, rubella, and varicella immunizations due to an exception, including the children of the provider, the parents of the infant at risk shall sign a statement that the parents have been informed of the risk to their child. This statement shall be in the infant's file at the facility.
