Adequate supervision was not provided when an enrolled child was in the same bathroom as a non-enrolled child and children were alone. During that time a collateral reports the non-enrolled children and children from the YMCA SAP program were changing swim trunks in the restrooms and the non-member children were making fun of the enrolled children. Per collateral contacts, a volunteer provided a sharp object to a 7-year-old child and explained to the child that the object could be used for self-harm. The volunteer demonstrated to the child how they could cut their arm using the object. Per collateral contacts, a volunteer encouraged children to say vulgar things in front of their peers. Per collateral contacts, the program director was made aware of concerning behaviors between a staff member and children and did not step in to prevent the continued occurrence of the concerning behaviors. (g) (2) No volunteer shall be counted in the supervisory ratio unless the volunteer meets all the requirements of a group leader or assistant group leader and is designated as a group leader or assistant group leader by the program director.
