On 4/23/2026 from 4:06pm to 4:26pm a male child (DOB 3/12/2024) was left alone on the playground with no direct supervision while the two staff took the rest of the class into the building. Ratio was 0:1. On 5/5/2026 a female child (DOB 6/16/2022) was left in her classroom with no direct supervision for approximately one minute when the rest of the children were taken by the three staff to the playground. Director allowed same staff member to return on 7/1/2025 and work another morning shift alone with children, before pulling her aside to request she go for a drug screen.
