Substantiated incident: Supervision Concerns (Supervision Concern)

There were supervision concerns at this facility. The health department determined during investigation that an individual on-site did not have a background check. This also put the facility out of compliance with child to staff ratios. Resolution: The staff member in question has recently received their cleared and enhanced background check.
