Substantiated incident: Supervision Concern (Health and Safety, Supervision Concern)

There were concerns that a small child was left behind at a park while on a field trip with the day care. The health department and law enforcement confirmed that a 4-year old child was left behind at a park while on a field trip. The child asked bystanders for help locating the daycare staff, and when it was determined they had left, 9-1-1 was called. The child was returned to their parents and withdrew from this daycare. Resolution: The provider has been working with IdahoSTARS to improve supervision practices on and off site. Additionally, the provider has determined they will not be going on field trips anymore.
