    There were ongoing concerns of a small child coming home with bite marks on a regular basis. Additionally this child fell from their highchair and was taken to a doctor to assess for stitches in their mouth. Upon investigation, the health department determined that there is a substantial supervision concern in this child's classroom. The investigator observed staff not actively supervising children and confirmed that the child fell out of the chair but did not require stitches. Resolution: The provider agreed to keep more experienced aids in the classroom until the new experienced, lead starts. At a follow-up investigation, the health department observed teachers actively supervising the children and the hiring of a new lead teacher with 20 years of child care experience.
