Substantiated incident: Supervision Concerns (Supervision Concern)

There were ongoing concerns of a small child coming home with bite marks on a regular basis. Additionally this child fell from their highchair and was taken to a doctor to assess for stitches in their mouth. Upon investigation, the health department determined that there is a substantial supervision concern in this child's classroom. The investigator observed staff not actively supervising children and confirmed that the child fell out of the chair but did not require stitches. Resolution: The provider agreed to keep more experienced aids in the classroom until the new experienced, lead starts. At a follow-up investigation, the health department observed teachers actively supervising the children and the hiring of a new lead teacher with 20 years of child care experience. A serious injury was reported by the child care facility. During the investigation, it was confirmed a child slipped and fell on a slide at the park and hit his tooth. Resolution: The child was comforted and a compress was put on the mouth to stop the bleeding. Parents were called and picked the child up. Provider implemented new schedule changes to stagger class recess times, and required all staff to take Active Supervision TTA Training to better prepare teachers. There were concerns a child tripped on uneven pavement while playing outside at recess. During the investigation it was found that the child was taken to the emergency room and needed 8-9 stitches for a gash on their forehead. The health department verified that ratios and supervision were in compliance at the time of investigation and during the time of the incident. Resolution: Provider advised to ask the University to replace the uneven sidewalk to prevent future injuries.
