There were supervision concerns of children. During the investigation it was verified a child hid from staff members in the bathroom and two children inappropriately touched each other. Resolution: Provider agreed to ensure staff members are more diligent about checking the bathrooms and only letting in one child at a time. An incident report was completed and CPS contacted.
