This provider self-reported that a child was running and tripped causing then to hit their chin on a tabletop. After a review of footage, the health department determined that the incident did occur and the provider had taken all necessary action to report the incident to the parents and provide appropriate care to the child. Additionally, there were no supervision or health & safety concerns at the time of investigation.
