At time of follow up investigation, the parents of the child were notified and the child received medical attention. The involved staff member was reassigned to a different classroom and is required to attend infant safety training. Substantiated incident: Supervision Concern, Serious Injury (Serious Injury, Supervision Concern)

The provider self-reported that an infant had fallen out of a high chair onto their face. The health department performed an investigation and determined a staff member had unbuckled an infant who was in a high chair and then tended to another child in the classroom. The infant fell out of the highchair onto their face and shortly after began vomiting. Resolution: The parents were notified and due to the severity of the incident, parents picked up the infant and took them to their pediatrician for an evaluation.
