There were concerns of background checks not being current, child-staff ratios out of compliance, and booster seats being expired. At the time of investigation, the child-staff ratios were out of compliance in one room, the car seats were expired, and one background check was not in place. Resolution: Provider has obtained current background check for the one staff member, new booster seats were purchased, and a child was moved into a different room to bring the facility back in compliance for child-staff ratios.
