Resolution: Due to numerous substantiated incidents and lack of ability to remain in compliance, this provider's Daycare License was Summarily Suspended.     There were concerns that the facility is frequently over child/staff ratios and that the facility was not up to health & safety standards. During the investigation, the inspector confirmed that this facility was significantly out of child/staff ratios. The cleanliness of the facility was noted to be relatively clean and did not pose any concern to the inspector. Resolution: A staff member arrived on scene to put the facility back into ratio compliance. The Department issued a Notice of Concern Letter in regard to the supervision concerns that were substantiated. The health department performed a follow-up investigation and no new violations noted.
