There were concerns that a staff member hit a child in their care and possibly other victims that were not reported. During the investigation, it was verified a staff member had been mutually let go over concerning multiple mishandling of negative behaviors. While investigating, the health department watched footage and determined that this staff member grabbed a child by the arm and drug them across the floor. Resolution: The provider agreed to have regular staff meetings to discuss acceptable behavior when dealing with discipline and what is not acceptable. There were concerns that this facility is consistently out of staff/child ratios. Additionally, there were concerns about a loft being unsafe for children and staff not reporting injuries to the families. During the investigation is was determined that a child did get injured on the loft and it was not reported to parents until pick-up. Additionally, a garbage can was easily accessible and children were getting into them. Resolution: The health department discussed supervision training with provider which training was conducted with staff on proper supervision at a later time.
