This provider self-reported that a child had escaped from the play yard through a hole in the fence. During investigation, the Health department verified the child was able to leave the facility through a hole in the fence and was found a few minutes later in a parking lot of a neighboring business. Parents were notified. Resolution: The hole in the fence was fixed the same day. The Health Department confirmed the repair and it appeared sturdy.
