   Idaho health & safety criterion failed: 3. Child-Staff Ratio

   At time of investigation, child staff ratios in the baby room were not in compliance. Staff member corrected on site and one child was moved to to a different room, making all rooms in compliance with provider to child ratio. Cited at a failed DHW health & safety inspection (Investigation) on September 11th, 2025. Investigation status: Not Resolved. Source: Idaho Child Care Check (DHW).

   Substantiated incident: Supervision Concerns, Safe Sleep Concerns (Supervision Concern)

   There were concerns that the facility is often out of compliance with child/staff ratios and has unprofessional business practices. During the investigation, is was found that a staff member did not have a valid CPR certification while providing care which put the infant room out of ratio. Additionally, two infants were observed to be sleeping in swings or bouncers. Due to past incompliances with safe sleep procedures, a follow up investigation will occur to ensure all employees are properly trained on safe sleep procedures and that handouts are posted in the facility. Resolution: Due to the infant room being over ratio, the provider moved a child to another room to bring the facility back into compliance. Safe sleep was also corrected on-site and staff were given information about ratios and safe sleep guidelines. At the follow-up investigation, there were no ratio concerns staff appeared to be well trained in safe sleep. Source: Idaho Child Care Check (DHW) substantiated incident report.
