    There were concerns that a toddler had hot sauce put in their mouth and restrained in a highchair as a form of punishment for biting. During the investigation staff denied the use of hot sauce but did confirm using highchairs as a restraint. During the follow-up investigation, there was no adult supervision inside the facility. There were several safe sleep violations, including a sleeping infant in a highchair with a blanket and an infant sleeping alone in a room. Additionally, children were inside the facility could not been seen or heard from where the staff was located outside. Resolution: After receiving notice of substantial concerns for safety of the children, the Department of Health and Welfare suspended and revoked this provider's license.
