At the time of the visit, one staff member is missing their employment history form. Correction: At the time of the visit, all three BAS staff members' employment history checks were compliant with HAR §17-801(a)(5) Background checks.

HAR §17-896-6 — Insurance coverage policy

At the time of the visit, the program does not have a current liability insurance on file with DHS. Correction: On October 31, 2025, the school's secretary emailed the worker a copy of the program's updated liability insurance, expiring on July 1, 2026.

At the time of the visit, all staff members were missing their employment history forms.

HAR §17-896-17 — Staff-child ratio

The facility is licensed for 40 children, ages 5 years old to 13 years old. On May 6, 2025, this worker observed 8 children enrolled and 3 children present with 1 program leader: One 2 year old, one 7 year old, and one 11 year old. Correction: The worker received a signed permission form from the parents of the 2-year-old child, giving the high school relatives of the child custody of the child. The after-school program does not oversee the child.

At the time of the visit, one staff member was present. That staff member did not have current pediatric first aid/CPR certification. Correction: The worker verified the first aid/CPR certification for William Tong, expiring 9/3/2027.
