(Section 18.3, number 3) Staffing Requirements [SR] Comments: 3-12 [SR] Class 3: No Signed CF-FSP 5337 Form [SR] Violation Detail: The operator and/or substitute did not sign a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form. (Section 18.3, number 3) At the time of this LFCCH inspection, this document was missing from a substitute's file. Technical Assistance was provided to the provider by explaining to them that this document needs to be filled out and kept in all personnel files. The provider was also informed that they can email the inspector this completed document. Compliance Comments At the time of the LFCCH License Renewal Inspection, it was observed that the Annual 10 Hour Inservice Log was missing for the provider and that one substitute was missing their log as well. For another substitute, it was observed that they were missing the training certificates for their Annual 10 Hour Inservice log. Technical Assistance was provided to the provider by explaining that they have until the end of December (12/31/2025) to email these documents to the inspector. 04.
