(Section 18.4, number 2) At the time of this LFCCH License Renewal Inspection, the 5-year employment history check was observed to be missing from the provider and substitutes files. Technical Assistance was given to the provider by explaining to them that they need to have a 5-year history employment check form completed for themselves and their substitutes. Compliance Comments At the time of this LFCCH License Renewal Inspection, this standard was observed to be in compliance. The provider as well as their substitute's all had their necessary training completed. 04.
