The operator did not have evidence that a transportation log was maintained for all children transported. FDCH/LFCCH Handbook, Section 2.4.2 Large Family [SR] Comments: During the time of time of inspection, Licensing Specialist observed that Provider did not have the transportation log for the month of February. FDCH/LFCCH Handbook, section 2.4.2 Transportation Log A log must be maintained for all children being transported in the vehicle or on foot away from and/or to the premises of the home. The log must be retained for a minimum of 12 months. The log must include each childs name, date, time of departure, time of arrival and the signature of the driver verifying all children were accounted for during the visual sweep. A health and safety deficiency having a low potential for harm to the children in care was observed. FDCH/LFCCH Handbook, Section 7.6, A Health Requirements [SR] Comments: During the time of inspection, Licensing Specialist observed that the swing set in the outdoor play area was loose/shaky and a screw was loose. Provider advised that she will not allow the children to use it. FDCH/LFCCH Handbook, section 7.6 Outdoor Time, Fencing and Play Area Requirements I. All homes must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. All playground equipment, if provided, shall be securely anchored, unless portable or stationary by design, in good repair, maintained in safe condition, and placed to ensure safe usage by the children. Daily attendance was not maintained to account for all children in care. FDCH/LFCCH Handbook, Section 8.4 A and B Children's Records [SR] Comments: During the time of inspection, Licensing Specialist observed that three children were not signed in. Licensing Specialist observed Provider sign the three children in during the inspection. This standard was brought back into compliance during the time of inspection. Children were not being adequately supervised in that [employee left alone with children]. FDCH/LFCCH Handbook, Section 6, G, H, I, L (Section 2.1 Health and Safety, Page 3) Large Family [SR] Comments: During the time of inspection, Licensing Specialist observed Employee alone with six children, infant through four years. The Owner/Operator was not in the home when Licensing Specialist arrived. Licensing Specialist observed the children running around the room and climbing on chairs and tables. The children were throwing blocks and toys around which hit other children. Licensing Specialist observed the children hitting and pulling on one another. The children would cry when another child would hit or throw toys at them. Licensing Specialist observed the Employee trying to manage the children's behavior but could see that it was difficult for her to do alone. Licensing Specialist conducted a fire drill with the employee and the six children. Licensing Specialist observed that it was difficult for employee to transport two infants, a one-year-old, a two-year-old, three-year-old and four-year-old outside for the fire drill. Employee was unable to complete the entire evacuation route for the fire drill because the two infants were too heavy for her to carry, which also made it difficult to manage the other children. Employee would not be capable of responding to the needs of all children in the event of an emergency. The Owner/Operator returned to the home 40 minutes after the arrival of Licensing Specialist and was able to assist with managing the behavior of the children. This standard was brought back into compliance during the time of inspection. FDCH/LFCCH Handbook, section 6 Supervision A. The operator shall remain responsible for the supervision of the children in care and capable of responding to emergencies and the needs of the children at all times.
