Potentially harmful items such as BB guns, pellet guns, knives and/or sharp tools were not in a locked area or were accessible to children in care. FDCH/LFCCH Handbook, Section 7.2 (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the time of inspection, Licensing Specialist observed scissors and a sharp knife being stored in an unlocked drawer in the kitchenette which was accessible to the children in care. The kitchenette is located in an open area of the playroom and the children walk past it as they enter the home. Licensing Specialist observed Provider remove scissors and knife and put them out of reach of children. This standard was brought back into compliance during the time of inspection. Child care personnel did not wash their hands with soap and running water and thoroughly dry after assisting a child with toileting or diapering or following personal hygiene procedures. The operator's substitute plan did not include the name, address and telephone number of the designated substitute. FDCH/LFCCH Handbook, Section 3.2, B Large Family [SR] Comments: During the time of inspection, Licensing Specialist observed that the Provider did not have required information for substitute on file. FDCH/LFCCH Handbook, section 3.2 Substitute The operator must have a written plan to provide at least one substitute, 18 years of age or older, to be available on a temporary/emergency basis. A. The substitutes information (including name, date of birth, telephone number, address, anticipated number of hours worked and whether or not this person substitutes for another home) must be provided on the CF-FSP Form 5133, Application for a License to Operate a Family Day Care Home, which is incorporated by reference in 65C-20.008(1), F.A.C. B. The written plan must be kept current and include the name, address, and telephone number of the substitute. C. Any changes to the plan must be reported to the licensing office within 5 working days. All hours worked by the substitute must be documented in writing and maintained for 12 months. D. Substitutes may not work for the operator more than 40 hours per month on average over a 6-month period in any single home for which they have been identified as the designated substitute. E. Large Family [SR] Comments: During the time of inspection, Licensing Specialist observed the substitute in the home with the children did not have background screening documents on file for Licensing Specialist to review. FDCH/LFCCH Handbook, section 4.1 Initial Screening Operators, household members (adults and children 12 to 17 years of age), substitutes, volunteers and Large Family Child Care Home employees must have a level 2 background screening clearance from the Department prior to obtaining a license, residing in the home, employment, or volunteering unsupervised with children. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws.
