The following furnishings/equipment in disrepair: [refrigerator was observed to not be working properly and was unplugged so it can thaw and be repaired. ]. Physical Facility [SR] Comments: At the time of this FCCH Routine Inspection, the provider informed the inspector that the refrigerator had quit working overnight and that they had unplugged the refrigerator and removed all the food so that the refrigerator could thaw and be serviced. The provider also informed the inspector that they fed the children food that did not need to be refrigerator for breakfast. (Muffin and fruit) as well as they served the children milk that did not require refrigeration. Technical Assistance was provided when the inspector informed the provider what the temperatures of the refrigerator and freezer need to be and that the refrigerator needs to be repaired as soon as possible. The inspector also observed the milk to make sure it was not expired. Compliance Comments At the time of this FCCH Routine Inspection, the inspector observed this standard and found it to be in compliance. The temperature within the FCCH was observed to be 73 degrees. A hazard was observed in the outdoor play area that poses a threat to the health, safety or well-being of the children. (Section 9.7, number 1) Physical Facility [SR] Comments: At the time of this inspection, there was several nails observed to be sticking out of the fence near the outdoor chalk board. Per the provider, the nails were used as hooks for pails. Provided TA that she may use the heads of the nails as the hooks but they should be higher up so they are out of the reach of children. Nails were removed at the time. Violation clear. Provider is in compliance. 16. Sleeping equipment (cots, beds, cribs, playpens, mats) were not spaced a minimum of 18 inches between individual napping spaces. (Section 9.4, number 2) Safety/Health [SR] Comments: At the time of the inspection, it was observed the children napping on the cots were not a minimum of 18 inches from one another. The inspector communicated to the provider technical assistance--that children must be a minimum of 18 inches from each other during napping and away from furniture/shelving. The provider stated where she would place the children's cots for nap time instead, to meet requirements. Nap time concluded while the inspector was present. As per standards: A minimum distance of eighteen (18) inches shall be maintained on all sides between individual napping spaces. Napping spaces shall not be designated in kitchens, bathrooms, utility rooms, or garages. The family day care home did not have immunization records for an animal or pet [dog] observed. (Section 9, number 10) Safety/Health [SR] Comments: At the time of the inspection, it was observed by the inspector during reviewing files, that the expiration date for immunizations of a pet dog was not available. Technical assistance was given that the provider must have proper immunization documentation for the dog with expiration dates--that an invoice would not suffice. As per standards: Household pets, animals or fowl, must be free of disease, and properly immunized, if immunizations are available for the type of animal, pet or fowl. There was no documentation that parents had received written notification of an animal observed on the premises. (Section 9, number 10) Safety/Health [SR] Comments: At the time of the inspection, written parental notification of a pet in the home was missing for one of the children's files (for A.G.). The provider later emailed the inspector a completed/signed parental notification of a pet paperwork for the child. As per standards: Parents must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flier, a notification statement, or a statement included in the childs enrollment form.
