The home's outdoor play area was not adequately fenced, in that there were holes or missing slats in the fencing, or fencing was unsecured. (Section 9.8, numbers 1 and 2) Physical Facility [SR] Comments: The fence surrounding the back playground area, underneath the fence is rotting which leaves an open space about 3inches . The area needs to be secured to ensure safety from any rodents. Technical Assistance given: Playground Area must be secure at all times for children safety.
