There was no documentation that parents had received written notification of an animal observed on the premises. Comments: 34-4 At the time of this inspection observed 2 children's file (S.T. and R.D.) with no authorized letter in files stating a dog in home. Dog (Rufus) expires 2-4-2028. TA: Advised provider that written notification of dog in home should be in each child's file. Compliance Comments At the time of this inspection observed fire drill log: Done on 10-13-25 at 4:30pm with 1 adult and 3 children with seconds to clear out back door. Conducted fire drill at the time of this inspection at 12:25pm with 1 adult 3 children and 1 FDOH inspector with 53 seconds time to clear out front door. Fire extinguisher last serviced on 11-2024.
