06. Background Screening Requirements FDCH/LFCCH Handbook, Section 4 (06-01)

Required background screening was missing for [ a juvenile family members, 12 and older (an FDLE check)]. 402.313(3) F.S. and FDCH/LFCCH Handbook, Section 4.1 Comments: During the inspection, the Licensing Specialist observed that the Family Day Care Homes Clearinghouse Employee/Contractor Roster was current. During the time of the inspection, the Licensing Specialist observed a Juvenile Record Search was not completed for one child household member. During the time of the inspection, the Licensing Specialist provided technical assistance, per the Florida Statutes: Child care personnel in family day care homes shall be subject to the applicable screening provisions contained in ss. 402.305(2) and 402.3055. For purposes of screening in family day care homes, the term includes any member over the age of 12 years of a family day care home operators family, or persons over the age of 12 years residing with the operator in the family day care home. Members of the operators family, or persons residing with the operator, who are between the ages of 12 years and 18 years shall not be required to be fingerprinted, but shall be screened for delinquency records. 10. Toxic Substances, Hazardous Materials and Hazardous FDCH/LFCCH Handbook, Section 7.2 (10-01)

Toxic Substances and/or Hazardous materials including cleaning supplies, flammable products, and poisonous items were accessible to children in care. FDCH/LFCCH Handbook, Section 7.2 Comments: During the time of the inspection, the Licensing Specialist observed Clorox toilet cleaner on the back of the toilet, 3 laundry detergent containers were lined up by the wall next to the toilet. All chemicals were accessible to the children in care. During the time of the inspection, this was resolved by the Operator moving cleaning supplies to an area blocked from childrens reach. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:A. All areas and surfaces accessible to children shall be free from toxic substances and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. These items, as well as knives, sharp tools, BB guns, pellet guns and other potentially dangerous hazards, shall either be stored and in a locked area or must be inaccessible and out of a childs reach. B. All potentially harmful items including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials must be labeled and used according to manufacturers recommendation. C. Narcotics, alcohol, or other impairing drugs/paraphernalia must be kept inaccessible to children at all times. D. Cleaning must not take place while rooms are occupied by children except for general clean-up activities that are part of the daily routine. General cleaning refers to cleaning necessary to maintain a sanitary environment but that does not pose a hazard to children, such as wiping the table after lunch, soaking toys in a tub on the countertop, sweeping. 23. Diapering Area Clean and Sanitized FDCH/LFCCH Handbook, Section 7.18 (23-04)

Items unrelated to diaper changing were stored in the diaper changing area or placed on the diaper changing table. FDCH/LFCCH Handbook, Section 7.18, B Comments: During the time of the inspection, the Licensing Specialist observed a dirty diaper, toys, and a pacifier were on the diaper changing pad. During the time of the inspection, this was resolved by the Operator removing the toys and pacifier and throwing the diaper away. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:When children in diapers are in care, there shall be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. A. The diaper changing area shall be located separate from the food preparation, food service and feeding area. B. Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall they be placed on the diaper changing table. 09. Animal Vaccinations FDCH/LFCCH Handbook, Section 7.1 (09-01)

The operator did not have documentation of current vaccination records for the pet observed.Vaccinations are available for this type of animal. FDCH/LFCCH Handbook, Section 7.1 Comments: During the inspection, the Operator advised that there is one dog on the premises. During the inspection, the Operator provided the Licensing Specialist documentation of the dogs vaccinations for Bordetella and Distemper which are both expired. No record for rabies vaccinations were available for the Licensing Specialist to observe. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook: Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the child's enrollment form. 31. Immunization Records FDCH/LFCCH Handbook, Section 8.1 (31-02)

The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)].FDCH/LFCCH Handbook, Section 8.1 A and B Comments: During the time of the inspection, the Licensing Specialist observed 1 child had expired immunization records. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. B. If the custodial parents or legal guardians fail to provide the above required documentation within 30 days of enrollment, the home shall not allow the child to remain in the program.
