The provider failed to maintain written documentation for the number of hours a substitute worked in their home each day for the preceding 12 months. FDCH/LFCCH Handbook, Section 3.2, C Staffing Requirements [SR] Comments: During the inspection, the Provider failed to provide the Licensing Specialist with written substitutes working hours for past 12 months. During the inspection, the Licensing Specialist provided Technical Assistance (TA) per FDCH Handbook: 3.2 Substitute The operator must have a written plan to provide at least one substitute, 18 years of age or older, to be available on a temporary/emergency basis. C. Any changes to the plan must be reported to the licensing office within 5 working days. All hours worked by the substitute must be documented in writing and maintained for 12 months. 16. Appropriate, Safe and Sanitary Bedding FDCH/LFCCH Handbook, Section 7.8 (16-02)

The bedding available was not safe and poses a threat to the health, safety or well-being of a child in care. FDCH/LFCCH Handbook, Section 7.8, A, B and D Health Requirements [SR] Comments: During the inspection, the Licensing Specialist observed that napping cots with fitted sheets on them were stacked and stored inside the childrens restroom. The Licensing Specialist also observed childrens blankets stored inside the restroom. Some blankets were placed in individual bins; however, several of the bin lids were not properly closed. Additionally, the Licensing Specialist observed two crib mattresses, each with a fitted sheet on them, stacked together inside one crib. The Provider explained that the mattresses were placed together because the other cribs had been moved to improve organization. The Provider stated that the linens would be washed once a week. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. FDCH/LFCCH Handbook, Section 7.10, A Health Requirements [SR] Comments: During the inspection, the Licensing Specialist observed one baby bouncer that was broken, with interior foam exposed. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: 7.10 Toys, Furnishings, Equipment and Plumbing A. All parts of the home, both indoors and outdoors; including the furnishings, equipment, toys and plumbing shall be kept clean, and sanitary, free from hazards, in an orderly condition and in good repair at all times. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. Meals and snacks supplied were not age appropriate and posed a choking hazard. FDCH/LFCCH Handbook, Section 7.14, F. Health Requirements [SR] Comments: During the inspection, the Licensing Specialist observed that the lunch served to the children (chicken and banana for age-one children) was not cut into age appropriate sizes. During the inspection, this has been resolved. After being reminded, the substitute immediately cut the food into age appropriate pieces. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: 7.14 Food and Nutrition F. Foods that are associated with young childrens choking incidents must not be served to children under 4 years of age, such as, but not limited to, whole/round hot dogs, popcorn, chips, pretzel nuggets, whole grapes, nuts, cheese cubes/sticks and any food that is of similar shape and size of the trachea. Food for infants must be cut into pieces 1/4 inch or smaller, food for toddlers must be cut into pieces 1/2 inch or smaller to prevent choking. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: 7.13 Food Preparation/Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. C. Permanent or stationary playground equipment must have a ground cover or other protective surface under the equipment that provides resilience and is maintained to reduce the incidence of injuries to children in the event of falls.
