    Additional documentation describing the symptoms that would require the prescription and/or non-prescription medication used on an as needed basis to be administered was not on file.FDCH/LFCCH Handbook, Section 7.24, A and 2.4.3, B Comments: During the time of the inspection, the Licensing Specialist observed a tub of Aquaphor, a tube of Desitin, and Infant Tylenol did not have written authorization from the custodial parent/guardian. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook: A. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the child care personnel who gave the medication. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given.
