09. Animal Vaccinations FDCH/LFCCH Handbook, Section 7.1 (09-01)

The operator did not have documentation of current vaccination records for the pet observed.Vaccinations are available for this type of animal. FDCH/LFCCH Handbook, Section 7.1 Comments: During the inspection, the Operator advised that there is one dog on the premises. During the inspection, the Licensing Specialist observed the vaccination records for the dog has expired since 02-06-2026. 7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the childs enrollment form. Documentation of current immunizations must be available for review upon request by the licensing authority. During the inspection, the Licensing Specialist observed that no written notice was posted to inform parents or legal guardians that a dog is present on the premises. 13. Indoor Play Areas FDCH/LFCCH Handbook, Section 7.5 (13-02)

Indoor play areas were not in good repair. FDCH/LFCCH Handbook, Section 7.5, B Comments: During the inspection, the Licensing Specialist observed two broken chairs in the child care room that had been repaired with transparent tape. During the inspection, the Licensing Specialist advised the Operator that tape cannot be accepted as permanent repair or used to secure items. During the inspection, this has been resolved. The Operator removed the broken chairs from the child care room and replaced them with new plastic chairs. During the inspection, the Licensing Specialist provided Technical Assistance (TA) per FDCH Handbook: 7.5 Indoor Play Areas B. All areas of the home including the play areas shall be in good repair, clean and free from litter, nails, glass, and other hazards. 14. Outdoor Time, Fencing and Play Area Requirements FDCH/LFCCH Handbook, Section 7.6 (14-02)

Outdoor play areas were not in good repair. FDCH/LFCCH Handbook, Section 7.6, A Comments: During the inspection, the Licensing Specialist observed one broken black and white three-wheel toy motorcycle with sharp edge in outdoor playground. During the inspection, this has been resolved. The Operator removed the broken toy motorcycle from the outdoor playground. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: 7.6 Outdoor Time, Fencing and Play Area Requirements A. Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards. During the inspection, the Licensing Specialist observed a large number of unknown red and black insects in the outdoor play area, including on the play equipment and turf. The Operator advised the Licensing Specialist that the insects appeared due to the recent rain. The Licensing Specialist advised the Operator to ensure the outdoor area is checked for insects before children go outside for play to prevent children from being baited. Pest control services should be contacted if necessary. If pest control is scheduled, it must not occur during operational hours or while children are present. Additionally, medication authorization forms must be obtained before applying any insect repellent to children in care. 16. Appropriate, Safe and Sanitary Bedding FDCH/LFCCH Handbook, Section 7.8 (16-03)

The bedding available for children in care was not sanitary. FDCH/LFCCH Handbook, Section 7.8, A Comments: During the inspection, the Licensing Specialist observed that childrens nap blankets were stored in direct contact with one another. During the inspection, this has been resolved. The Operator replaced blankets in each childs storage bin. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: 7.8 Appropriate, Safe and Sanitary Bedding A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. 10. Toxic Substances, Hazardous Materials and Hazardous FDCH/LFCCH Handbook, Section 7.2 (10-01)

Toxic Substances and/or Hazardous materials including cleaning supplies, flammable products, and poisonous items were accessible to children in care. FDCH/LFCCH Handbook, Section 7.2 Comments: During the time of inspection, the Licensing Specialist observed the laundry door open with washing powder accessible to children in care. Also, there was a bottle of hand sanitizer on the living room side table accessible to the children in care. During the inspection the owner removed the hand sanitizer and placed in the laundry area and locked the laundry door. During the time of inspection, the Licensing Specialist provided technical assistance: "A. All areas and surfaces accessible to children shall be free from toxic substances and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. These items, as well as knives, sharp tools, BB guns, pellet guns and other potentially dangerous hazards, shall either be stored and in a locked area or must be inaccessible and out of a childs reach. B. All potentially harmful items including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials must be labeled and used according to manufacturers recommendation. C. Narcotics, alcohol, or other impairing drugs/paraphernalia must be kept inaccessible to children at all times. D. Cleaning must not take place while rooms are occupied by children except for general clean-up activities that are part of the daily routine. General cleaning refers to cleaning necessary to maintain a sanitary environment but that does not pose a hazard to children, such as wiping the table after lunch, soaking toys in a tub on the countertop, sweeping. The bedding available was not safe and poses a threat to the health, safety or well-being of a child in care. FDCH/LFCCH Handbook, Section 7.8, A, B and D Comments: During the time of inspection, the Licensing Specialist observed the infant napping in his crib with a blanket and toys. This citation was brought back into compliance when the owner removed the blanket and toys from the crib. During the time of inspection, the Licensing Specialist provided technical assistance: "A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs, play yards, and playpens used for infants must have tight fitting sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. D. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard."
