A prescription or non-prescription medication, specifically [desitin, diaper rash cream, shampoo, cologne ], was dispensed without written authorization from the custodial parent or legal guardian. FDCH/LFCCH Handbook, Section 7.24, A and L (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the time of the inspection, the Licensing Specialist observed 2 tubes of Desitin, 1 tube of diaper rash cream, baby cologne, and shampoo in the diaper changing table cabinet with no written authorization from the custodial parent/guardian. During the time of the inspection this was removed by the Operator removing the desitin, diaper rash cream, shampoo, and cologne from the diaper changing table, they will be sent home with children today. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the child care personnel who gave the medication. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given.
