FDCH/LFCCH Handbook, Section 4.1, L Comments: During the time of the inspection, the Licensing Specialist observed 1 household member was not added to the Clearinghouse roster and 1 household member was not removed. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:L. The employer/owner/operator must add substitutes, employees and household members to their Employee/Contractor Roster within ten days of when the individual has received a child care eligible result Employer/owner/operator must add an end date within ten days of termination for individuals on the Employee/Contractor Roster in the Clearinghouse when employment terminates, or a household member is no longer residing in the home. Furnishings, toys, equipment or plumbing were not clean, safe and maintained in good repair and pose a threat to the health, safety or well-being of the children in care. FDCH/LFCCH Handbook, Section 7.10, A Comments: During the time of the inspection, the Licensing Specialist observed a bike with a cracked seat which was missing a piece of the plastic, a bike missing the pedals which exposed rusted metal, and lounge chairs which were rusted on the bottoms. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:A. All parts of the home, both indoors and outdoors; including the furnishings, equipment, toys and plumbing shall be kept clean, and sanitary, free from hazards, in an orderly condition and in good repair at all times. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. During the licensure year, the operator failed to conduct monthly fire drills utilizing the approved fire alarm system or smoke detector at various dates and times when children are in care. FDCH/LFCCH Handbook, Section 7.21, D Comments: During the time of the inspection, the Licensing Specialist observed the time to clear all either has the number 1 or a checkmark, no specification for which route taken as each month is documented by a check mark, no initials from Operator for each drill. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook:D. A current attendance record and parent/guardian contact information must accompany child care personnel out of the home during a drill or actual evacuation, and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. A prescription or non-prescription medication, specifically [Aquaphor, Baby powder, 2 Nystatins, Mupirocin, Petroleum jelly, Baby lotion ], was dispensed without written authorization from the custodial parent or legal guardian. FDCH/LFCCH Handbook, Section 7.24, A and L Comments: During the time of the inspection, the Licensing Specialist observed 1 tube of Aquaphor, 1 bottle of Baby powder, 2 tubes of Nystatin, 1 tube of Mupirocin, 1 jar of Petroleum jelly, and 1 bottle of Baby lotion were dispensed without written authorization from the custodial parent or legal guardian. During the time of the inspection, this was resolved by the Operator removing the medications from the daycare room and returning them to the parents/ legal guardians. During the time of the inspection, the Licensing Specialist provided technical assistance, per the FDCH Handbook: A. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the child care personnel who gave the medication. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care personnel must never administer a medication that is prescribed for one child to another child. L. Use of diaper creams and insect repellant may only be utilized with written permission from parents/guardians. Manufacturer instructions should be followed."
