FDCH/LFCCH Handbook, Section 8.2 Comments: During the inspection, the Licensing Specialist observed one present childs file only contained expired physical check form. During the inspection, the Licensing Specialist provided Technical Assistance per FDCH Handbook: 8.2 Student Health Records A. The operator is responsible for obtaining for each child in care a current, complete and properly executed DH 3040, Student Health Examination form, incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, or a signed statement by an authorized professional that indicates the results of the components of the form are included in the health examination from the custodial parent or legal guardian, within 30 days of enrollment. C. The Student Health Examination form or signed statement is valid for two years from the date the physical was performed and must be on file as long as the child is in care. 09. Animal Vaccinations FDCH/LFCCH Handbook, Section 7.1 (09-03)

The operator did not have documentation that custodial parents or guardian are given written notification of animals on the premises. FDCH/LFCCH Handbook, Section 7.1 Comments: During the inspection, the Operator advised that there is one bird and four chickens on the premises, all of animals kept in cages in the backyard. During the inspection, the Licensing Specialist observed that no written notification was posted for custodial parents or guardians. The Operator advised that she had only verbally informed the custodial parents or guardians. During the inspection, the Licensing Specialist provided Technical Assistance per FDCH Handbook: "7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the child's enrollment form. 13. Indoor Play Areas FDCH/LFCCH Handbook, Section 7.5 (13-01)

Indoor play areas in the home were not clean and free from litter, nails, glass, and other hazards. FDCH/LFCCH Handbook, Section 7.5, B Comments: During the inspection, the Licensing Specialist observed unknown black substances on the windowsill of the childrens restroom. The Operator advised the Licensing Specialist that she had cleaned earlier this morning and suggested those substances may be blown by the wind as the window was opened. Additionally, the Licensing Specialist observed accumulated dust on the window AC unit in the child care room. During the inspection, this has been resolved. The Operator wiped down the windowsill of the children's restroom and window AC unit in the child care room. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: "7.5 Indoor Play Areas B. All areas of the home including the play areas shall be in good repair, clean and free from litter, nails, glass, and other hazards." Outdoor play areas were not in good repair. FDCH/LFCCH Handbook, Section 7.6, A Comments: During the inspection, the Licensing Specialist observed rusted nails on two kids powered cars and on one red three-wheel scooter located in the outdoor play area. During the inspection, the Licensing Specialist observed a broken blue toy car in the outdoor play area. During the inspection, the Licensing Specialist observed that the handle of the screen door leading to the indoor area was broken and had been wrapped with tape. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: "7.6 Outdoor Time, Fencing and Play Area Requirements A. Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards." The home did not have an operable corded telephone available during hours of operation.FDCH/LFCCH Handbook, Section 7.12, A Comments: During the inspection, the smoke detector was tested and was operational. The ?re extinguisher certi?cation is valid until 10-2026. During the inspection, the Licensing Specialist observed that the Operator had a corded phone on the premises but had not installed it. As a result, the Operator was unable to demonstrate that corded phone was functional. During the inspection, the Licensing Specialist provided TA per FDCH Handbook: "7.12 Telephone, Lighting, Temperature and Ventilation A. The home shall have at least one operable corded telephone." Compliance Comments During the inspection, the Operator advised that she is a Food Program participant providing meals and snacks to the children that are in care.
