32. Health Records FDCH/LFCCH Handbook, Section 8.2 [SR] (32-02)
The Student Health Examination was not acceptable in that: [ An applicable part of the record was not complete]. FDCH/LFCCH Handbook, Section 8.2 (Section 2.1 Health and Safety, Page 3) Comments: Specialist reviewed all 5 children files. On one form the doctor did not put a date next to his signature. The provider notified the parent while specialist was present. The Family Childcare Home Handbook states on page 47:8.2 Student Health Records A. The operator is responsible for obtaining for each child in care a current, complete and properly executed DH 3040, Student Health Examination form, incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, or a signed statement by an authorized professional that indicates the results of the components of the form are included in the health examination from the custodial parent or legal guardian, within 30 days of enrollment.
