Daily attendance was not maintained to account for all children in care. Comments: Attendance log showed that one child was not signed out on 5/15. In addition, the same child was present today, but not signed in. Another child was already signed out for today for 4:30 pm. Provider scratched out the time, so the parent could write in the actual time at pick up. Another child was picked up early for a dr appt but not signed out. Provider stated that the child will be returning. Provider wrote down the time the child was picked up. The provider stated that her corrective action plan is to review the attendance after children arrive and after they leave to ensure parents are completing the attendance correctly.
