An area(s) of the program facility was observed to not be in good repair. (Section 10, number 1) Physical Environment [SR] Comments: At the time of inspection observed, the lobby's wall next to the cafeteria had peeling paint on the floor from the wall's mural. Children were observed sitting at this area in the interim awaiting to enter the cafeteria to start afterschool. Technical assistance was provided; the facility should be maintained the peeling paint should not be accessible to children to reduce the incidences of illnesses. (Section 10.7, number 1) Physical Environment [SR] Comments: At the time of inspection, observed overgrown poison ivy on hedges along the entrance path to playground, where 711-year-old children frequently pass and play by. The presence of the poison ivy by the playground poses a hazard to children, such as allergic reactions. Technical assistance was provided--specifically notifying ASP director to place a work order for the removal of the poison ivy. At the time of inspection, ASP director created work order (on 5/2/25) for removal.
