   The facility did not have a fully-completed enrollment form (CF-FSP Form 5219) or equivalent form for the child(ren) in care. CCF Handbook, Section 7.3 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: During the time of the inspection, the Operator advised Licensing that there are currently 35 children enrolled. During the time of the inspection, Licensing monitored 35 of the 35 childrens records. During the time of the inspection, Licensing observed that 5 of children's files did not include the custodial parents/legal guardian's acknowledgment/consent on the enrollment forms regarding receiving discipline/expulsion policies, the child care facilitys food and nutrition policies that includes language on food safety and food allergens, authorized persons for pick-up and for child care personnel to have access to child's records. During the inspection, technical assistance was provided: Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. CF-FSP Form 5219 may be obtained from the licensing authority or on the Departments website at www.myflfamilies.com/childcare. A. Enrollment information shall be kept on file, current and available for review by the licensing authority. B. The enrollment information shall include, in writing, permission for the facility to release the child to any person(s) authorized or in the manner authorized by the custodial parent or legal guardians. The name, address, and phone number of authorized persons must be in the enrollment information. C. There shall be signed statements from the custodial parents or legal guardian that the child care facility has provided them with the following information: 1. The Departments child care facility brochure, CF/PI 175-24, Know Your Child Care Facility, which is incorporated by reference in 65C-22.001(7)(m), F.A.C. This brochure may be obtained from the Departments website at www.myflfamilies.com/childcare. Local licensing agencies may use an equivalent brochure approved by the Department. 2. The child care facilitys written disciplinary and expulsion policies. 3. The child care facilitys food and nutrition policies that includes language on food safety and food allergens. 4. Annually, during the months of August and September, the child care facility must provide parents/guardians of children enrolled in the facility with information detailing the causes, symptoms, and transmission of the influenza virus. To assist providers, the Department developed a brochure, CF/PI 175-70, Influenza Virus, Guide to Parents, which is incorporated by reference in 65C-22.001(7)(n), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com/childcare. 5. Annually, in the months of April and September, the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.CF/PI 175- 12, brochure, which is incorporated by reference in 65C-22.001(7)(x), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com . D. Enrollment information shall include parental/guardian consent for child care personnel to have access to childs records. Compliance Comments During the time of inspection, the Operator advised Licensing that there 5 employees. During the time of the inspection, Licensing monitored 5 of the 5 employee records.
