A complete CF-FSP Form 5131, Background Screening and Personnel File Requirements, was not on file for all employees. CCF Handbook, Section 7.4.1, A (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing Specialist viewed CF-FSP Form 5131 was missing from all personnel files. Corrected at the time of the inspection. Technical assistance was given regarding CF-FSP Form 5131. Provider stated that CF-FSP Form 5131 will be completed for each personnel and kept in file. Provider stated personnel files will now be located in the Human Resources Department at the facility. Per CCF Handbook, Section 7.4.1, A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. (page 62).
