12. Facility Environment CCF Handbook, Section 3.1 (12-01)

Areas of the facility were found to be in need of cleaning. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: During the inspection the licensing specialist observed areas that were in need of cleaning, The licensing specialist observed that there were dusty back splashes, bath room cabinet in 4 year old room is dirty, portions of floor in 4 year old room needs cleaning, and cabinet in age one room. The Licensing Specialist took pictures. The Licensing Specialist provided technical assistance from the handbook Section 3.1 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. The bedding available was not safe and poses a threat to the health, safety or well-being of a child in care. CCF Handbook, Section 3.6.1 Physical Environment [SR] Comments: During the inspection, the licensing specialist observed the sheets on the three empty cribs were not tight. The director tightened the sheets, and ordered new sheets for the cribs. The licensing specialist took pictures. The Licensing specialist provided technical assistance from the handbook Section 3.6.13.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. Section 3.6.4 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Comments: During the inspection, the licensing specialist observed that the signs outside the building do not have the current license number or name listed on the signs. I All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. The lock for the cabinet has broken allowing children to have access. The licensing specialist took pictures. The director corrected the issue by removing the cleaner bring the center into compliance. An employment history verification was not documented and on file. CCF Handbook, Section 5.1, D Record Keeping [SR] Comments: At time of the inspection, the Licensing Specialist observed two staff records did not have complete documentation of five years of employment history in the file. The Licensing Specialist took pictures. The director completed the phone verifications for the staff prior to the completion of the inspection. The Licensing Specialist provided technical assistance from the handbook 5.1, D The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. CCF Handbook, Section 4.2.3 and 4.2.5 (Section 2.1 Health and Safety, Page 3) Training [SR] Comments: At time of the inspection, the Licensing Specialist observed two staff record didnt have documentation of completing Safe Sleep training within 30 days of hire. The Licensing Specialist provided technical assistance from the handbook Section4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Provided technical assistance from the handbook Section 7.1, Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.
