A new in-service training record is required each fiscal year. The in-service training records for the previous two fiscal years must also be maintained at the child care facility for review by the licensing authority. Sec. 7-38(b)(4) and (5), Ordinance: Infants shall not be placed for napping or sleeping in any car safety seat, bean bag chair, bouncy seat, infant seat, swing, jumping chair, highchair, chair, futon, sofa, or any other type of furniture or equipment that does not meet the State Requirements for cribs and play yards. Infants who arrive at the facility asleep in a car safety seat must be immediately removed from the seat by the parent or the child care personnel and placed on their backs in a safe sleep environment. An infant was observed sleeping outside of a crib/play yard. 2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must include: (1.) Each childs name, (2.) The date and time of departure, (3.) Time of arrival at the destination, (4.) The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and (5.) The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). The facilitys log did not include all required information. 6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: Complaint alleges that child had a hand print on her leg that resulted in an open DCF case. FINDINGS: A complaint inspection was conducted on 9/18/2025 and a statement from the teacher (JA) and assistant teacher (MM)was received. Per the assistant teacher (MM), it was story time in the morning. The child would get picked up at 11:30AM. The incident happened teacher was reading a story. I was sitting on the rug with the children. She went under the easel. The easel began to fall so she held the easel with one hand and with the other hand she pulled the child back by the leg. At the moment, she didnt see a mark on the child so a report was not written. On the same day after the child was picked up, the police came and collected a statement from her. There were no other incidents that could have caused the mark on the child. Per teacher (JA), she had a big group and it was almost dismissal time when the incident happened. She was reading a book and the assistant teacher was on the carpet with all the kids. The child, (A.C.) was crawling everywhere and the teacher said stop and calm down many times. The child went under the easel and Ms. M.M. grabbed the child's leg and with the other hand tried to stop the easel from falling down on her. Then when she saw the child, the child had the mark of the assistant teacher's hand on her leg. She did not get a chance to tell the office or create an accident/incident report. When she came to notify the office, the child had already left and told mom that the teacher hit her. This complaint is substantiated. A non-compliance is being issued as an accident/incident report was not created and shared with the parents on the day of the incident. 41.
