3.8.4(A) and (C), Handbook: During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. A signed statement from the parent acknowledging receipt of the policy must be included in the infants enrollment file maintained at the facility. Diaper changing occurred on a surface that was not impermeable. Toilet/Bath Facility [SR] Comments: At the time of inspection, licensing monitor observed [1] changing pad in the 1yr old room to have holes and was not impermeable and [1] temporary changing pad that had been taped. Provider resolved by disposing of and replacing both pads.
