33. Food Protection and Service Group: Sec. 7-42, Ordinance and DCF Handbook (33-01)

3.9.6(A) and (D), Handbook: Breastmilk and formula must be handled in a sanitary manner at all times and according to manufacturers instructions and instructions by parents/guardians. Prepared bottles must be placed in the refrigerator immediately and used within 24 hours. Prepared bottles were not properly refrigerated and/or used within 24 hours. Food Service/Nutrition [SR] Comments: At the time of inspection the monitor observed two infant bottles locked in the snack cabinet. Per the teachers, the bottles were received earlier during the childs arrival time and provided to the children after lunch. Child care personnel stated bottle A was milk and a vitamin supplement; bottle B was regular milk. Licensing brought this concern to the teachers and program director reminding them of food safety requirements. The milk found in the infant bottles were discarded bringing this standard into compliance. Technical Assistance was provided for Food Protection and Service Group. 6.5(D), Handbook: In the event of an emergency, non-prescription medication that is not brought in by the parent or legal guardian can be dispensed only if the program has written permission from the parent or legal guardian to do so. The facility did not obtain written authorization prior to administering emergency non-prescription medication. Childrens Records [SR] Comments: At the time of inspection, the facility did not obtain written authorization prior to administering a vitamin supplements found in an infant bottle. Licensing brought this concern to the attention of the teachers and program director reminding them of medication requirements. Per program director, required written documentation will be received moving forward. The contents observed in the infant bottle was discarded bringing this standard back into compliance. Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision occurred when [see details below]. Ratios/Discipline [SR] Comments: At the time of inspection, the licensing monitor observed children sleeping within 4's classroom without staff personnel supervision. The licensing monitor brought this violation to the attention of the child care personnel on site and requested that [1] additional child care personnel from adjacent classroom move to bring classroom supervision requirement into compliance. 7-33(d), Ordinance: When children are napping, the child care direct supervision requirements set forth below shall apply in addition to the staffing ratio requirements: (1) If any of the napping children are under 12 months of age, there must be one child care staff member for every four children. (2) If any of the napping children are under 2 years but over 12 months of age, there must be one child care staff member for every six children. (3) If the napping children are all at least two years of age but none are three years of age or older, there must be one child care staff member for every 22 children. (4) If the napping children are all at least three years of age, there must be one child care staff member for every 30 children. During naptime, a staff-to-child ratio of [see details below] staff for [see details below] children was observed. Comments: At the time of inspection, the licensing monitor observed the 2-3's class group out of ratio during naptime. Ratio observed [1] child care personnel supervising [24] children, if the napping children are all at least two years of age but none are three years of age or older, there must be one child care staff member for every 22 children. Additionally, the 1's class group was observed out of ratio during naptime. Ratio observed [1] child care personnel supervising [12] children, if any of the napping children are under 2 years but over 12 months of age, there must be one child care staff member for every six children. 21. Outdoor Play Space Group: Sec. 7-37, Ordinance and DCF Handbook (21-06)

Sec. 7-37(b), Ordinance: The outdoor play space and any area adjacent to any outdoor play space that a child may have access to shall be free and safe from any reasonably foreseeable condition that has the potential of endangering or does endanger the health, safety, and welfare of children, including, but not limited to, the presence of hazardous materials and high-tension wires. The following hazards were observed: [SEE BELOW]. Comments: There was an area in the toddler playground that contained loose pebbles that pose a choking hazard to children.
