Sec. 7-38(b)(7), Ordinance: All cribs and play yards shall contain mattresses or mats that fit the dimensions of such cribs or play yards without any gaps and that are in compliance with the manufacturer's specifications for mattresses or mats to be utilized in such cribs and play yards. The crib/play yard mattress did not meet the requirements. Comments: At the time of inspection, a crib was observed with two stacked crib mattresses. Provider resolved by removing second mattress. Sec. 7-11.10(c), Ordinance: A completed application for renewal of a license must be submitted to the local licensing agency at least 45 days prior to the expiration date of the current license to ensure that a lapse in licensure does not occur. The facility failed to submit a complete renewal application 45 days prior to the license expiration. Comments: At the time of inspection, provider has paid relicensing fee but has not submitted the relicensing application. A late fee may be assessed at receipt of application. The providers license expires on 03/31/2025. 3.7(I), Handbook: Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. The restroom was found to be unsanitary. Comments: At the time of inspection, licensing monitor observed the bathroom in the PreK 3 room to be in need of cleaning. The garbage can was overflowing, and discarded paper towels were observed on the floor and in the tub. 21. Outdoor Play Space Group: Sec. 7-37, Ordinance and DCF Handbook (21-04)

3.5(A), Handbook: The outdoor play area must be clean and free from litter, nails, glass and other hazards. The following hazard was observed in the outdoor play space [trash in mid-section of playground]. Comments: Following the recommendation of CCLE management and to resolve a previous non-compliance, provider fenced off section of playground that held materials for delayed playground installation. The playground installation is still pending, and the packing materials are degrading. Additionally, the area now contains additional trash, broken toys, and broken furniture. Provider must clean the area, limiting the space to properly stored items related to the playground installation only. Materials should be properly stored or covered and trash, including degrading packaging, should be removed from the area to discourage pests and vermin. Failure to maintain the area will result in the removal of space from the licensed capacity. If capacity is removed, area should remain clean and maintained to discourage pests and vermin. 3.6.1(G) and (H), Handbook: Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. Bedding and linens shall not be stored in the bathroom, unless stored in cabinets. The storage of bedding and/or linens was not sanitary. Comments: At the time of inspection, licensing monitor observed teacher's belongings, a basket, children's dress up clothing, and nap time linens stored on top of the cots.
