An area(s) of the facility was observed to not be in good repair. (Section 2.1(A) Health and Safety, Page 3) Facility/Storage [SR] Comments: The two water faucets in the bathroom sink of the 2-year-old classroom were not working properly. One faucet did not have running water, and the other was shut off due to a leak under the sink. In addition, two toilets in the 4-year-old classroom bathroom were out of service. 11. Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [Children observed sitting on carpet while the teacher watched he phone]. Ratios/Discipline [SR] Comments: Complaint alleges that 12/01/2025 at approximately 5:30pm, Parent walked into the daycare center to see a young black lady she's never seen before sitting on the counter by the door on her phone as her daughter sat across the room on the floor staring at the wall all alone soaking in pee to the point that the diaper is falling apart. Parent was upset that the young lady was not properly supervising the child. FINDNGS: A walkthrough of the classroom was done and the following was observed; Three children were observed sitting on the carpet watching media content on a tablet and the teacher was leaning on the shelf, watching her phone. When the young lady saw me, she quickly put down her phone and stood up. Per the assistant director, the teacher may have been updating the parent app. The teacher's parent app was checked and none of the children's pages were updated since around 2:45PM and teacher was observed watching her phone at around 4PM . 2.6(C ), Handbook: The written plan also must include scheduled activities that do not include the use of electronic media for children under two years of age. Electronic media was used for children under two (2) years of age. Indoor/Outdoor Space [SR] Comments: During the course of a complaint inspection related to diapering and inadequate supervision, the following non-compliance was observed; - Three children in the 1y/o classroom were observed sitting on the carpet with a tablet in front of them watching media content. The teacher was in the same area, but leaning half her body over the shelf, watching her phone. TA was given that the use of electronic media does not include children under two. 19. Sec 7-30(a)(3), Ordinance: All child care personnel who provide care to infants at child care facilities shall, prior to caring for such infants, successfully complete the most current training course approved by the local licensing agency relating to providing an infant safe sleep environment, as identified on the local licensing agencys website. The owner of a child care facility must ensure that all such child care personnel are in compliance with the training requirements of this section, and that such training is renewed every 2 years, in the same manner as provided for the initial training. Successful completion of the required training must be evidenced by a certificate, or any equivalent documentation, issued by the applicable training organization in the name of the individual who completed the course and indicating the date the course was completed. Documentation evidencing compliance with the training requirements under this section shall be included in the individual's personnel file maintained at the child care facility. Child care personnel working with infants did not have the required training. Comments: The two childcare personnel observed providing care to infants did not have a current Safe Sleep training. 04. Sec. 402.305(4)(a), Florida Statutes: Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. The facility has not been inspected and approved by the local fire department as required.
