Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision occurred when [see below]. Ratios/Discipline [SR] Comments: At the time of inspection, licensing monitor observed a teacher in the hallway carrying a child. While conducting ratio, it was observed that the teacher in question was assigned to the infant room. [3] children were left alone, ages infant-1yr. A Notice of Violation will be issued. 3.6.4(A), Handbook: Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. The crib/play yard sheets were not appropriate. Indoor/Outdoor Space [SR] Comments: At the time of inspection, licensing monitor observed [2] crib sheets that were too large. One was held around the mattress with large file clips. The other crib was observed with two stacked crib mattresses. Item is partially resolved, as provider removed the extra crib mattress. Sec. 7-35(c)(2), Ordinance: Manual or electrically powered commercial water coolers may be used to meet the indoor drinking fountain requirements. Electrically powered commercial water coolers may be used to meet the outdoor drinking fountain requirements. A water cooler was found to be out of water. Comments: At the time of inspection, licensing monitor observed the water cooler used to meet indoor drinking fountain requirements to be empty. Cooler is blocked by materials and inaccessible to children in care. Per provider, fountain has not been in use. This standard was revised on 11/25/2025 after supervisory review to clarify language and issue corrected due date to align with renewal period. 15. 3.9.5(B), Handbook: Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. 3.9.6(B), Handbook: The provider must make sure all formula and food brought from home are labeled with the childs first and last name. The provider is responsible for the label; therefore, if the label is not completed by the parent, the facility staff must put the label on when the formula or food is received. Bottles, sippy cups or food were not labeled and/or stored properly. 2.5.4(A), Handbook: All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. Provider did not obtain inspection by a qualified, certified mechanic and retain documentation in the vehicle.
