The owner of a child care facility must ensure that all such child care personnel are in compliance with the training requirements of this section, and that such training is renewed every 2 years, in the same manner as provided for the initial training. Successful completion of the required training must be evidenced by a certificate, or any equivalent documentation, issued by the applicable training organization in the name of the individual who completed the course and indicating the date the course was completed. Documentation evidencing compliance with the training requirements under this section shall be included in the individual's personnel file maintained at the child care facility. Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision was observed in the [infant room]. Ratios/Discipline [SR] Comments: During the course of a complaint inspection on 8/19/2025, the following non-compliance was observed; The Owner/Director went to the front office with an infant in her arms, leaving two children, an infant and a 1y/o child alone in the infant room. The owner stated " I know I'm in violation" when she entered the office. I didn't understand what she meant right away until I asked her why she didn't leave the infant in her room so that she could have free hands. She stated that the infant teacher was out to lunch so then I realized that she left the infants alone. I proceeded to the infant class to keep an eye on the babies until she returned. She returned to the class shortly after. A Notice of Violation is being issued. 08. Ratios of Personnel to Children Group: Sec. 7-33, Ordinance and DCF Handbook Not Monitored Compliance Comments Complaint alleges that on 08/12/2025, the owner's son/employee physically abused the children at the daycare. He was thumping them in the head for moving around on their mat. The children cried and yelled. He curses at the children. There are concerns for the level of force used on the small (2- to 3-yearold) children. One of the children is terrified of the employee to the point he literally shakes. FINDINGS: A complaint inspection was initiated on 8/18/2025 and completed on 8/19/2025. A statement from the owner and son was received. Per the director, she was on vacation last week, from 8/8/2025 to 8/16/2025. She left her two sons in charge during her absence. On Tuesday, 8/12/2025, there were 3 staff present, T. Smith (son), A. Moore and S. Swan. The other son was not on premises during naptime. T. Smith was in the mixed group of 1y/o to 4y/o covering naptime alone until 1pm, because the other teacher, A. Moore, was in the infant room covering for the infant teacher who was out to lunch from 12pm - 1pm. Per the owner's son, T. Smith, He denied both allegations regarding thumping the children on the head and cursing at them. He stated that he tells them to go to sleep but he does not touch them. He just tells them that if they behave then they can miss the next field trips. He stated that sometimes he guides them back to their beds, he never did anything of an aggressive nature. After the DCF investigation, he called the parents and told them that they would close due to the open investigation because he wanted the director to be here when the children returned. He stated that he called the technician to see if he could get access to the computer. He thinks that they are having problems with the computer. The owner contacted the technician during my inspection who stated that someone from the school contacted him on Friday, 8/15/2025, and said that they were having difficulties accessing the videos. He believes that that the videos are not available for review. He is scheduled to come out to check the system on Wednesday, 8/20/2025. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. Ratios/Discipline [SR] Comments: During the course of a complaint inspection regarding discipline, the following non-compliance was observed; On 8/18/2025, the mixed group of 1y/o to 4y/o children was observed out of ratio. The following ratio was observed; infants 1:4, 1y/o to 4y/o 1:10. Owner was in the front office from the moment i entered until i observed the out of ratio in the class and asked her to return to her class. She told me that she was int he office assisting me but I informed her that she must remain in the classroom. On the date in question, 8/12/2025, as per the attendance and statements provided by the staff, there were a total of 13 children ranging from ages 1 to 5 in the room adjacent to the office. The 13 children were alone with one staff member from 12 to 1pm. There were a total of 3 staff present that day, and 2 absent. One being the owner (out of town) and the other being a male employee/family member was not onsite during that time. TA was provided on maintaining adequate ratios.
