All seats, safety restraint devices, and fixtures must be maintained free of rips and tears and in good repair at all times. Vehicles must be cleaned and sanitized routinely. Comments: The following repairs are needed NEW VAN 1 of the seatbelts in the last row appears to have been cut and is fraying. The provider has been informed that the seatbelt cannot be used until it has been repaired.
