3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: Cracks and rust were observed in some areas of the outdoor equipment. Sec. 7-45(b)(2), Ordinance: All trash, debris, and dirt must be removed from the vehicles daily. All seats, safety restraint devices, and fixtures must be maintained free of rips and tears and in good repair at all times. Vehicles must be cleaned and sanitized routinely. The facility failed to maintain the vehicle as evidenced by [see below]. Comments: A ripped seat belt strap was observed on the third row of vehicle VIN # ending in 5232. 3.1(A) and (G), Handbook: All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation.Cleaning must not take place while rooms are occupied by children except for general clean-up activities that are part of the daily routine.This does not include cleaning with hazardous materials or any cleaning which poses a risk of slipping or falling.Areas of the facility were found to not be clean, sanitary or well maintained Facility/Storage [SR] Comments: The following areas were found in need of repair: - Children's bathroom sink needs to be cleaned. - Kitchen cabinets need to be cleaned. - Two seat belts buckle covers were observed cracked in vehicle vin# 1741 and one in vehicle vin#5223. - Torn seat fabric exposing metal structure was observed in vehicle vin # 5223. - Edges of the carpet were detached from the ground and accumulated trash was observed in vehicle vin# 1364. - The plastic driver's seat panel was observed detached in vehicle vin# 1364. - The water fountain at the handwashing sink should be disabled.
