   Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [playground incident]. Ratios/Discipline [SR] Comments: Allegation: An incident occurred when child fell on the playground and busted his lip. His grandmother, who works at the school, went to pick him up due to my work schedule and found blood on his shirt and a visible injury to his lip. When asked if he had told the teacher, he said he had, but again, she did nothing. I brought these concerns to the director repeatedly. Eventually, they reviewed the security footage and confirmed that everything the child described was true. Despite this, no action was taken against Ms. Gwen. In fact, the footage showed her on her phone while my son was injured and bleeding notifying her about his lip . On May 6, another troubling event occurred. The childs aunt picked him up, and the child's teacher loudly told her, Yall need to get on him! He kicked and punched a little girl in the stomach. We were very surprised to hear this and did not believe it matched his character. FINDINGS: An on-site inspection was conducted on 5/14/2025 and statements from the director and the teacher (A) in question was received. Per the director, regarding the first incident on the playground the child fell on the playground and another staff member brought the child to the grandmother and spoke with the parents at pick up. An incident report was created but the provider did not have the parents' sign the report. The provider stated the injury was small and not visible. Regarding the incident on May 6th, the provider stated that she was not present when the incident occurred but that she reviewed the footage. The children were in a blind spot of the camera and all you could see is the other child coming around the corner walking up to the teacher, holding her stomach. Video footage of the May 6th from 4:20PM to 4:45 PM was reviewed. The following was observed: The teacher assigned to the supervision of the children was on the phone, sitting on a picnic table. She appeared to be scrolling on the phone screen for the duration of the video clip that was reviewed. I observed the two children walking to the right side of the playground and exited camera view. A short while later, the child walks into camera view holding her stomach, and walks toward the teacher. The teacher looks up and called the boy over. The footage shows the teacher talking with both children gesturing them to hug each other. Soon after, the children returned to play This complaint is substantiated for inadequate supervision as the teacher was engaged in her phone during playground time. Ratios of Personnel to Children Group: Sec. 7-33, Ordinance and DCF Handbook Not Monitored Compliance Comments Allegation: Complaint alleges that child stated that teacher (A) called him special needs and said she was often mean to him, frequently sending him to time out. She took everyone to the playground and made me stay inside. The teacher ignored his request to use the restroom until it was too late and he peed himself. Other children have hit him, and when he reports this to Teacher (A), she just looks at him and doesnt intervene. FINDINGS: A statement from teacher (A) was received. She stated that there is a special needs child in the classroom that likes to hold the other teacher's hand. The other children get jealous and want to do the same. The teacher informs the children as a group that there are children that have special needs. Teacher A also stated that she does not exclude the child from activities, on this instance she had to move children, including him to VPK to maintain ratio. With regards to the incident where the child wet himself, the staff are not aware of such incident. Teacher stated that she has no problems with the child in question. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.    Sec. 7-44(a), Ordinance: In order to be accountable for the children in care, to facilitate the release process, and to ensure the safety of the children in its care, the child care facility must have, implement, and follow written procedures for the release of children. There was no written release procedure on file at the facility. Comments: ALLEGATION: On May 7th at around 11:00 a.m. a child was released to an unauthorized person who was an employee of the school and the child's maternal grandma. FINDINGS: The child's enrollment records were checked for compliance. The grandmother is not listed as an authorized individual on the child's pick- up list. Although the grandmother is not on the pick up list, she is an employee of the school and had been authorized to pick up on multiple instances. In this instance, the written release procedures were not followed. This complaint is deemed substantiated.    3.9.6(G), Handbook: Due to the extreme risk of choking, solid foods, including cereal, may not be given in bottles or with infant feeders to children with normal feeding habits unless authorized by a physician. Solid foods may not be fed to an infant younger than 4 months of age unless directed by a physician. Solid foods must be of a safe consistency and must be developmentally appropriate for the age and developmental ability of the infant. Solid food and/or cereal was fed to an infant under 4 months of age in a bottle and/or without authorization from a physician. Food Service/Nutrition [SR] Comments: Due to the extreme risk of choking, solid foods, including cereal, may not be given in bottles or with infant feeders to children with normal feeding habits unless authorized by a physician. Per the director, she was unaware [Child A] was being served cereal with milk in their bottle and the policy. Moving forward the director and teachers will monitor bottles and will notify parents physician authorization is required for bottle accommodations.
